MONROVIA — The husband family of the late Ntombikayise Innocentia Khumalo, widely known as Toni Khumalo Jackson, has formally called for a joint autopsy and international forensic investigation into her death, as tensions emerge over plans to repatriate her remains to South Africa.
In a second official statement issued on March 21, the Jackson family disclosed that the South African Embassy near Monrovia has requested the release of Toni’s remains for evacuation to South Africa—reportedly before the Liberian government concludes its autopsy process.
According to the family, the request was communicated through the St. Moses Funeral Parlor, which is currently handling arrangements.
Family Insists on Full Investigation Before Burial
While acknowledging the involvement of South African authorities, the family stressed that no final decision on burial or repatriation should be made until the cause of death is conclusively established.
“At the same time, the Jackson Family considers it essential that a comprehensive and transparent determination of the cause of death be concluded before any final arrangements are made,” the statement noted.
To ensure credibility and public confidence, the family is requesting that the investigation be conducted jointly with the participation of:
- The South Africa Government
- Representatives of the Jackson family
- An independent international forensic pathologist
The family believes such a process would “help establish the truth” and provide closure not only to Mr. Samuel P. Jackson but also to the Khumalo family.
Marriage and Legal Standing Highlighted
The statement also underscored that the late Toni Khumalo was legally married to Mr. Samuel P. Jackson, a point the family appears to be asserting in the context of ongoing discussions over custody and final arrangements.

Despite the emerging disagreement, the Jackson family emphasized its willingness to work with all relevant stakeholders, including the South African Embassy.
“We remain committed to constructive engagement and full cooperation… in the interest of transparency, mutual respect, and due process,” the statement said.
